Question:
Does anyone have letters that they would be willing to share with me?

For the diet history, just write down the different diets you've been on, dates, how much weight lost, etc. I don't think I'd worry too much about exact dates or amounts lost. I wouldn't make anything up, but most of us have not kept exact track of what we've done. As for the reasons you want to have the surgery, just write down what is in your heart. It will be much better coming from you than if you copy something from someone else. Good luck.

Email me as well, if you'd like. I have 3 appeal letters that your more than welcome to look at and use what you can out of it. My policy, unfortunately, has an 'exclusion' to it's policy but I'm still going to fight it. For now, I'm going in as a self-pay (approximately $25,000.00)but well worth it to me. I will continue to fight them, with an attorney in hand, to get back my money.
