It's different depending on your contract. Call the HR office and ask them if the benefits schedule is online or if you can pick one up. Good luck.

They are all different here are the ones I have: http://www.regence.com/ This would cover Oregon, Washington, Idaho and Utah If you have BCBS are your insurance company call customer service and ask them for their website.
