Does anyone know the criteria for approval from medicare??

Medicare does not pre approve any service. Everything is based on Medical Necessity. You may want to call your local Medicare office @ 1-800-382-1274 (for Doctor service) and 1-800-853-1419 (for Hospital service). They may be able to go over coverage for those services.
