Anyone got info on getting approved through avmed hmo

Anyone having information about getting approved through avmed hmo after being told this surgery is not covered, even though willing to get anything needed to prove medically necessary....desperate    — Camille C. (posted on May 8, 2003)

I had my lap band 7/21/06, Dr. Cywes only had to submit once. I have Avmed HMO for State employees.
