I have bcbs fl ppo does 5 yr. requirement of MO have to be in a row or can it ?

be from 5 or more years ago ,for example I have records that show I was morbidly obese in 1996 but from 1997,1998 I would have to search for my records through hospt. etc...Does anyone know if they accept it that way or do I have to have them in a row?Thanks for any help..    — marie M. I wouldn't worry, I would send in what you have. The biggest thing is the 12 month supervised diet.
