Please post the reason you were denied; that will help narrow things down to the right kind of appeal letter.

Look in the library under appeals. Someone on one of the questions will tell how to get to where there is an appeal letter on this site. I think it might be under "your journey "link" above. Then look under "insurance troubles". I think there is an appeal letter listed on there you could use.
