Question:
Are there any states which have mandated insurance coverage for surgical treatment?

As a matter of fact the state of Georgia has just mandated that all insurance companies must cover this surgery if it is found to be medically necessary. If you would like a copy of the new law email me privately and I would be more than happy to get you a copy. [email protected]

I'm working on the legislature in North Carolina. They have had a committee for a few years that have been studying why NC has a high rate of fatality from heart attack and stroke. I've targeted the individuals on the committee with letters stating that the mortality rate and health care cost would both be reduced by covering treatment for morbid obesity. The link for the legislature is http://www.ncga.state.nc.us/html1999/billInfo/billInfo.html

In Maryland a law has just been passed making it illegal for insurance companies to deny coverage of bariatric surgery - no more blanket exclusions. This is SB522 for anyone who would like to get more information on this bill. It was signed by the governor 5-15-01 (I think I have the date right). It goes into effect 10-1-01. I confirmed this information with my State Senator's office today.
