I am only 23 years old, but I went back to age 10 when my mom tried to help me lose weight...I listed things like like counting calories, low fat, and many other diets including slim fast, water pills, atkins, grapefruit, etc. I listed the year and how many times I tried them and how much weight I lost and how much I regained. Also, I listed the various exercise programs I used/joined to try and lose weight. LOL....then I went to the extreme and gave them a 3-day sample menu of what I currently eat including snacks and drinks...I don't know how much all of that helped, but my Dr faxed it along with his papers to the ins company. I was approved in 8 days with NO Dr supervised diets. E-mail me if you want to chat with me more.

i went back as far as i could remember...& thats a long time lol. i have been overweight/obese ALL my life. i remember being 5 years old & my mom buying me 6X dresses or chubbies. (isnt that an awful thing child clothes manuf. do to a child?) my first formal attempt at dieting was at age 12 when my mom made the dr put me on diet pills. & it followed thru the years. i had to estimate the years, dates & weight lost & regained. who can keep track of how many weight watcher meetings i attended & the dates. i associated my attempts with landmarks in my life. eg: my sweet 16, my 18th birthday & appendectomy. my wedding, the birth of my children etc. take ur time & sit in a nice comfortable chair. i know the memories of the 'yo-yo years' r painful but im sure this is the information ur insurance company is looking for. good luck!
