Has anyone had surgery approved under UHC Choice plan?

I have been completely denied with UHC Choice of NC. it will probably depend on whether there is an exclusion clause. I have a strong exclusion clause and they will not pay no matter what. I know there are many people who have gotten coverage from their UHC. there are many different policy plans.
