What Are The Specific Medicare Criteria for 'Medical Necessity'?

Where can I find the precise listing for the Medicare Criteria of Medically Necessary? I checked on www.medicare.gov but couldn't find anything about Bariatric Surgery, weight loss surgery or gastric bypass. I have Medicare and because I have diabetes, hypertension and high cholesterol I was approved! I was told by my doctor that Medicare doesn't require pre-approval either. I had my consult last week and will be getting a surgery date this week.
