Sylvia, did you ever get the information you where looking for? If not try Providence Bariatric center? The number is 577-7570, ask for Lori Montez or Liz Moak, they will get you started. They will explain to you what you can do and how to go about it. They will also let you know if your insurance covers this surgery.
