how do you post those scales with the weight loss goals , appointments etc?

Finally getting closer to finishing all my pre surgery appointments. How do you put those stats and the scale icon with your posts?

I think its on your profile page under health tracker. I had trouble finding it too! Hope I'm giving you the right directions!
