Hi Teresa! Yes i've applied for OOC, they faxed them in at my doc's office. The ohip fax number is 807-475-1427. Their phone number is 807-475-1354. Hope I am some help! Everyone on here is incredible!! They are so much help, i don't know what i'd do without them!! jackie

I got my OOC papers filled out, signed and faxed to OHIP, and then when I got home and checked the mail i got my papers from Dr. L!!! How exciting!! Everything's falling into place!! I have to remind myself to not get too excited!! I filled out my Dr. L papers and I will fax them tomorrow...............the ball is a rollin"!!!!!!!
