Time to go back to basics. Get protein in first, chew very well and set down your fork in between bites. Also do not drink while you eat.

What's caused the gain?  Are you eating more food?  Or eating food higher in calories?  Exercising less?

What Dawn said + get up and move - one hour's exercise at least 5 x per week ...

Log everything that passes your lips on something like MyFitnessPal.com - make sure your protein intake is sufficient and eliminate the white or processed carbs

Head to your Dr to be sure there is no underlying medical reason for the gain (perhaps a medication side effect?) before your appointment you might want to chart your input (food & drink) and output (exercise) till you visit him/her and take that data with you... usually charting will tell you the story (less cals, less fats, less carbs, more exercise, more protein, more water, smaller portions, etc.) but it's good to get everything checked out.

Better start with exercises of light to moderate intensity, do some cardio workouts like running, swimming and biking and combine it with a good diet plan that suits with your exercise program/ routine. Diet and exercise always go together. Always remember that. :)
