Does anyone know of an insurance in indiana that will pay for gastric bypass surgery 100%?

You mean that will pay 100% of the cost? No. I don't think any will. Generally you have a copay or have to pay a percentage. Medicaid may cover the full amount, if you qualify for that. Otherwise, you will probably be responsible for a portion of the cost.
