Bariatric Registry

I'm not a happy camper right now. My Doc told me he sent my referral in January and looks like he didn't send it to the right place. Grrrrr 

Does anybody know where my Doc should be sending my referral? I'm going to see him today and want to make sure it is sent properly this time.

my doc sent all my info to the bariatric center of Toronto west General Hospital. Dr Jamie Cyriac was my surgeon. I had surgery on tuesday april 15 2014 and was sent home the next day. other than gas pain i am feeling great.
