Call the number on your insurance card and ask if your plan covers bariatric surgery. If it does, ask what steps you need to take.
Please note: I AM NOT A DOCTOR. If you want medical advice, talk to your doctor. Whatever I post, there is probably some surgeon or other health care provider somewhere that disagrees with me. If you want to know what your surgeon thinks, then ask him or her. Check out my blog.
I don't know if you need a referral from a primary care physician or not. When you call, they will tell you. they can tell you if you must do a physician supervised diet before they will pay for weight loss surgery and any other requirements of your plan.

You will probably have to call them. Many of the web sites don't break down specialists into enough detail to find a bariatric surgeon that way.
I do know that David Schumacher at Sycamore Hospital in Dayton accepts Caresource. Or used to, anyway.

Yes, if they require a certain number of months of pre-op diet (not all plans do), it has to be medically supervised so they won't start counting the months until a doctor starts supervising it.
