Call the number on your insurance card and ask if your plan covers bariatric surgery.  If it does, ask what steps you need to take.

I don't know if you need a referral from a primary care physician or not.  When you call, they will tell you.  they can tell you if you must do a physician supervised diet before they will pay for weight loss surgery and any other requirements of your plan.

Start by calling Caresource or going to their websites.  They should have a collection of participating doctors like other insurors do.

You will probably have to call them.  Many of the web sites don't break down specialists into enough detail to find a bariatric surgeon that way.

I do know that David Schumacher at Sycamore Hospital in Dayton accepts Caresource. Or used to, anyway.

Ok so i need to call Caresource tomorrow and ask, but if they do and need me to go on a plan does that mean i half to start all over because i have been on a diet for 4 months but not through a doctor, i am constantly on diets lol i just don't know where to begin

Yes, if they require a certain number of months of pre-op diet (not all plans do), it has to be medically supervised so they won't start counting the months until a doctor starts supervising it.
