At the TWH orientation they said that we would purchase it from them and it would be delivered to our door. They said that a credit card makes it all possible - without one you might run into some difficulties??

I bought it from my surgeon's office. Once they give you a surgery date they know what date you need to start the Optifast. When my date was scheduled I just picked it up at his office and paid in cash. I had to be on it for 2 weeks so I paid $200. I chose to get all chocolate and did very well on it. I lost 15 pounds in those two weeks.
I hope it goes as easily for you :)
