I was on insulin and was taken off of it when i did optifast.  My sugar were sometimes high but they were OK with that. Talk to your clinic.

I was taken off some of my meds and lantus was cut in half. Reading now 4.0 to 5.5 all the time. I was told that if sugers drop below 4 to cut lantus in half again.

I was taken off my secondary diabetes med but kept the metformin.  This was decided when they called me with my surgery and optifast start date.  My blood sugars were stable.  Not excessively high, mind you I was only drinking optifast....with SF jello and broth.

I'm not diabetic, but I am sensitive to artificial sweeteners. So instead of Optifast, the clinic has me on a modified liquid diet of Boost Diabetic (it's more tolerable for my allergy) with 1-30g scoop of protein. You might discuss this option when you speak to your doctor.
