Most insurance companies offer plans that cover it and plans that don't. You'll need to check your specifc policy to see what is covered.

It will usually be in the Summary Plan Description whether it is covered or not. In my case it was explicitly excluded. I had to pay for mine upfront. However, even if it's covered there are hoops to jump through. You or your doctor will have to call them to figure out what they require. I'm sure the people that go through them will tell you it's worth it. Just like I will tell you it was by far the best money I have ever spent and I am just over 2 months out.
