What is Going On??
If your belly is swollen, the 3 pounds is probably excess water. Also, 400-500 calories a day may be too low (and your body has dropped your metabolism in response to it).

I 2nd the calories too low concept. When I was at my last apt, about 6 wks out, I sais I was averaging 400-550 cal/day, and the doc said that was too low and your body may go into starvation mode & not lose. I would ask your doc's office how many calories you should be up to

The infection is likely preventing weight loss as well. When your body is fighting infection or trying to heal a wound, it can hold onto all of it's resources trying to build cells.
My nut says no less than 800 calories per day.
I would also try to boost my protein a lot during your healing phase, it will help.
You are still at 24 pounds, which is 4 pounds per week - so don't be too bummed.
Stay off the scale, drink more fluids, add a protein shake, shoot for more calories. Get well and be positive!!
