Ripped Open Pouch?

Help.  I am a RNY flop.  Had my surgery in 2007 and I've completely back slid the last few years.  I've not been doing my post op orders.  I'm drinking pop, overeating again, drinking alcohol too much, eating carbs to excess, not exercising.  I've got this new problem that just scares me.

I think I have thinned out my stomach and have a hole.  My whole abdomen has been bloated to new levels of pain I've not ever had.  I am bloated from my diaphragm to my pelvis.  It hurts to stand up straight.  My lower back feels pressure and pain.  Every time I eat any amount, I am having sharp pain.  All of this started on Monday night (about 5 days ago) after a big dinner and martini binge.

Is it possible my stomach contents are leaking into my abdominopelvic cavity?  Has anyone every heard of this happening?

My surgeon has moved and there are no bariatic specialists in my area.  I have no one to turn to.  I'm afraid to tell my husband what I think I going on.  What if I get septic?  I don't have time to be sick and take off of work.  I don't want another surgery.

Go to the doctor asap. To the ER even. If you think that could be a possibility don't mess around with waiting.

If your stomach contents had been leaking into your abdominal cavity since Monday, I doubt you'd be able to post this.  You'd be very, very, very sick.  Still, you need to get checked out ASAP.  I'd go to the ER.

Please note: I AM NOT A DOCTOR.  If you want medical advice, talk to your doctor.  Whatever I post, there is probably some surgeon or other health care provider somewhere that disagrees with me.  If you want to know what your surgeon thinks, then ask him or her.    Check out my blog.

I have to think the odds are in your favor for it not being anything too serious, but if you did rip your pouch, wouldn't that qualify as a medical emergency? If you are in as much pain as you say you are, my recommendation is to seek medical help sooner rather than later. A general practitioner could get the ball rolling.

Well, you could do nothing.  You can call in sick from the ambulance taking you to the hospital - or better yet, have the surgeon call for you.

Seriously, if you are in as much pain as you describe, you should go to the ER.  You've had pain for a week with no improvement.  We can "guess" what's wrong......we cannot diagnose you.  When it comes to you health, it's up to you to be pro-active.  Unfortunately, we can't wish our problems gone.  Waiting can make things worse ...... Don't wait.

Pleas take care of yourself!

You need medical help not a support forum.

It would be wise to get yourself to the ER.  Forget being afraid about how your husband feels!  No matter what state you are in, this is the US.  There will be a doc to help you.  Get there!

You might feel better if you cut out the drinking and follow a post-op lifestyle.

I can honestly tell you that if you are actually asking this question not from the ER, but from home, then you have not ripped your pouch open.  My pouch actually ruptured (ulcer at anastomosis) 2 weeks ago and the pain was more than debilitating.  I could barely breathe.  I had to have emergency surgery that day due to the rupture and went into septic shock which led to kidney failure.  So, why wait for that to possibly happen?  I didn't have time for it either.  Too bad my stomach didn't ask me about my schedule.  Please go to the doctor, any doctor.  There are tests that can be done that do not require a bariatric specialist.  I actually went to the ER in my city at 3 AM thinking I had a kidney stone and they did a CT scan.  No kidney stone and hardly any kidney function.  They immediately ordered an ambulance ride to a more equipped hospital & surgeon that was waiting when I got there.  I was told I would have died (DIED) if I would have waited.  Don't wait until you are in a position that surgery is required.  Take control of the situation now!  And, for what it's worth, you are not a flop.  Just get back to the basics.  You still have restriction.  Get checked out now.

If there's a hole in your stomach, you would be full of hydrochloric acid. That stuff can pickle steel, and I doubt you'd be standing if it was running amok in your guts.

As others have said, go to the ER if you're in "life-threatening" pain after 5 days. Or try a gastroenterologist, who specializes in all things stomach. Or call your GP and ask them to refer you to somebody.

echoing the others, you should see your pcp or go to the emergency room. If you had ripped a hole in your pouch 5 days ago, with no care, you would be dead now from sepsis.  I am thinking more likely ulcer. Gl
