Question about profile

can someone explain to me how I can add my doctor and the type of surgery I am going to get under my avatar picture where it shows how long I have been a member on this site? I cannot for the life of me find out how to do this. I am pre op.

 Go to My OH, then on the left hand side,click on history. 

      
Month 1= 19#     Month 2=11#    Month3=8#  Month 4 =10#
