I had my surgery in 2012 I lost over 130 pounds but in the past year I have gain 30 pounds between being sick, depression, and surgery. I have no idea what would be the best way to get back on track because it seem like each time I do all the old ways start to creep back in. I really need advice on the subject.

The first thing would be to start honestly tracking calories/protein/carbs again.  See where you are and start making better food or portion choices to get in a range you want to be in.

generally upping protein and reducing carbs is a good way to start.

ditch the empty calories from sweets and treats, sugared beverages, and alcohol.

I agree that the best thing to do is start tracking every bite you put into your mouth to see what you're actually consuming.  Then, work on increasing your protein, decreasing your carbs, increasing your water, and cutting out liquid calories that aren't something like a protein shake.
