 EATING
*64+ oz of water a day* DRINK 1OZ EVERY 15 min. (sip sip sip!!)
Eating every two hours for 60-70 g of protein (go for 100g of protein a day)
No drinking during or after meals for 30 minutes No tomato products for 3 months
No bread, pasta rice for 6 months (then 4 oz allowed per day)

No straws, carbonation, drinking fountains, alcohol & limited caffeine & smoking
Limit carb intake to 5-8 g at one sitting (40g a day)when counting carbs, do not use net carbs  
MEDICATION
Calcium should be taken in 4 doses around 300mg 2- morning, 2-evening
IRON (CARBONYL) (18-27mg) Take with Vit C at bed time at least 2hrs from calcium.
b12 sublingual
300-500mg a day
biotin
don’t eat anything after taking pills wait 10 mins in between pills wait 10mins before taking a second pill  
OTHER
30-45 mins of exercise daily (break down two 15min walks)
no pregnancy for 2 years
no NSAIDS or oral contraceptives

The ASMBS recommends the following:

A multi that has 100% of the RDA of stuff in it. Take two a day.

1500-2000 mg calcium citrate.  Make sure it's citrate, not carbonate.

18-27 mg iron, unless your labs show you need more.

B12.  You can use a sublingual, 350-500 mcg per day (unless your labs show you need more), a patch once a week, a nasal spray once a week, or shots once a month.

They say a B complex is optional. 

Many people also need D3.  If you have not had your vitamin D level tested, it's a very good idea to do so.



After RNY, B12 MUST be taken in injection, sublingual, or nasal inhalant form.  It requires binding to an enzyme called intrinsic factor (IF) in order to be absorbed.  IF is secreted in the part of the stomach that was bypassed.  Therefore it is inaccessible to the RNY patient.

DO NOT take calcium and iron within 2 hours of each other.  They are both absorbed using the same cellular receptor sites.  The receptor sites like calcium better, therefore the calcium will be absorbed and the iron will be excreted in the feces.

DO NOT eat or drink any of the following within 2 hours of taking iron: dairy, eggs, fiber, tea, coffee, red wine, grapes, or spinach.  They each contain substances that bind with the iron.  The iron will then be excreted in the feces.

DO take the CARBONYL form of iron.  It does not have as many GI side effects as does the ferrous sulfate form.  Feosol offers a carbonyl iron that is 45 mg pure iron that is available at your local grocery or pharmacy.

DO take vitamin C with iron.  It enhances the absorption of iron.  If iron upsets your pouch, take it with a meat snack.  This will buffer the pouch and and enhance the absorption of the iron.

Calcium should be taken in divided doses NOT to exceed 500-600 mg at a time.  The body just cannot absorb more than that at a time, no matter your WLS status. 

Calcium CARBONATE is not readily absorbed by ANYONE, no matter their WLS status.  As someone who has had WLS, we really should be taking calcium CITRATE.  It is better absorbed.  I take Citracal Petites.

preop test results

  I spent ALL day at RI hospital doing my testing.  I had my Gall bladder ultrasound today and the tech was acting kind of weird. it was pretty obvious she found something wrong when she refused to tell me my results and said “Dr. Roye will talk to you about this”.  It made me a bit nervous because i wanted to know how bad it was and she didnt say anything which made it even worse.  So I left the office thinking the worse and pressed on to next appt which was CCK HIDA scan. I walked in the office, registered and waited in the room for 45 mins to find out there’s no need for the scan because my ultrasound showed that i had GALLSTONES  the size of golf balls. UGH! You have to be kidding me! then that D*** sentence again “Dr. Roye will talk to you about this”. I don’t want to wait a week or two for answers i want to know NOW! But whatever i lived what them for this long I’m not going to let it ruin my day. i walked out of the office upset but optimistic that everything will be okay and moved on to my sleep study follow up hoping for good news. when i walked into the room the RNP said i had sleep apnea. I stopped breathing 17x a hour and my oxygen dropped to 78.  As a result i have to use a machine at home for 4 weeks and most likely won’t be able to do the surgery until i was finished. At this time i was ready to cry nothing was going my way and i was getting a little discouraged. Out of 3 appts not one told me what i wanted to hear. On my way home i received a call from the Hospital saying that there was a cancelation if i was available to take my pulmonary function test & ABG. I was a bit hesitant since my day was going the way it was but i went dragging my feet. After the testing the tech looked at me and told me that my testing went great and i wouldn’t have to do my ABG. WooWhooo finally some good news! Anyway, I’m proud i finished 4 out of 6 test in one day. All i have left is my Echo-Cardiogram and blood work which is scheduled for Thursday and i will be done with ALL my pre op testing. I’m ready for ALL the challenges that I have ahead of me, I welcome it with open arms because what doesn’t kill us does makes us stronger. read the ingredients carefully especially with over the counter remedies

Anti-inflammatory medications, used for pain, joint inflammation, or fever are usually irritating to the stomach.

 

 

Tylenol is OK.

 

Potassium Pills K- Dur, Klor-Con, Micro-K, KCl Steroids including

(Joint steroid injections or back injections are probably OK)

6 week pre op plan

figured i'd pass down some info that i found very helpful

 
week 6 - Read WLS for Dummies. As much as possible every day - take at least a little bit of a walk. It will strengthen you for post op walking.

Week 5 - Start your journal. Write your feelings about the surgery, about your obesity, about how this all feels. You won't want to forget - don't trust you own memory.

Week 4 - Keep walking. Even if it hurts and even if you don't want to. You will need that core strength and you will be walking the day of your surgery...so get used to it, friend!

Week 3 - Clean your kitchen cabinets. Get rid of the crap food. You won't want it and your family doesn't need it. Shop for your immediate post op diet, but shop "lightly" since a little food - even liquids - goes a long way and your tastes may change. Try some broth, herbal teas, sample protein packs, crystal light, SF popsicles, SF jello, and then refine it based on your doc's and nutritionist recommendations.

Week 2 - Clean the house. You won't feel like it for a while afterward. Shift your reading on OH to the GOOD posts. You need to have POSTIVE thoughts about the results that lie ahead! *organizing closet by size so it will be fun to get rid of sections at a time!*

Week 1 - Take "before" pics and measurements and put them in your journal. Pack for the hospital. Pack light - you won't need much of anything to "do" while you are there. They will keep you busy walking, sipping, and you'll snooze between. DO bring a chap stick, slip on tennies `for your walks (better support on hard hospital floors - crocs work too), and your own pillow for better rest and for the ride home.
