Help filling out papers

Hi All,I am just getting started filling out my papers for weight loss surgery. I have just started my 6 month's to see if I can lose weight, by my family Dr. I guess I am just afraid of what to answer on the papers, don't want to answer anything the wrong way, that would hurt my chances of being approved.
