Initially my GP faxed a form to the registry, after a few months we realized that she had faxed an old form (don't count on the central registry to contact your GP when something like this happens!) Once I realized, by calling the registry after a few months to find out they did not have a referral.
There is a current referral form on the registry site for GP's to print and complete.
I was fortunate once my referral was sent (my GP specifically requested Humber on the referral form! Typically you are sent based on your postal code to the appropriate centre) I called the Humber bariatric clinic almost daily to see if they had received my referral,and voila, one day the secretary said yes then booked me for the orientation that same week.
It worked the same for the other appt's I called to say I'd been to the orientation and would like to book the next appt's which she did.

I start 2 weeks of optifast Oct 22 , 2013 and am booked for a revision from lapband to bypass November 5 th. Some people choose to go on a waiting list to have their surgery faster. I chose not to, I need this time to prepare mentally and feel good about this choice.
