HELP - Revision on Gastric Bypass

I had surgery in 2001 and lost over 100+pounds.  I have kept it off for over 19 years and last year I started gaining weight.  Does anyone know if Anthem insurance will approve a revision and if so which revision will they do? Is it hard to get approval through them, if so what do I need to do?  My gastric bypass was approved by BCBS.  My pouch has not stretch (I don't know why it hasn't because now I am always starving).  HELP
