Jerry, Why don't you ask THEM what the criteria is. I had a terrible time getting my insurance company to tell me. Actually, they ended up deciding to approve my surgery...never would tell me what the exact criteria was....one person admitted that they were not allowed to tell! Ask to see (or talk to) the next person up the 'chain of command' if the person you are talking to refuses to answer your question! Good luck and God Bless!
