My doctor in stouffville had his office manager take care of it for me. it doesn't have to be the dr. themselves. maybe these docs don't know that. a lot of them are not very tech savvy so they don't want to do it thinking it will be a long drawn out process.

Referral June 2014/Orientation Humber August 2015/transfer to Guelph January 2016/

Orientation Guelph February 2016/Trio #1 March 2016/Trio #2 April 2016/

Post op nutrition class May 2, 2016/Meet the surgeon May 5, 2016/ Dr. Bhojani Surgery May 24, 2016

I'm in Durham. My Dr reluctantly did the referral. I'm below 40 BMI, so he said they probably wouldn't even consider me. Took three months for him to complete it and send it on. That was last May '14. Now I'm seeing the surgeon to get my date on October 15th.

I had the same issue.. 4 doctors..no go...finally got myself a permanent doctor who did it the same day I asked... Its crazy
