I don't know about dropping so low on calories.  If you do that, are you getting in all the protein and other nutrients you need?  Is it sustainable long term or will it lead to you eventually rebelling and eating more because you aren't getting enough?

A suggestion: try going to your RD at your surgery center and sitting down with the last 2 weeks of your food diary and reviewing.  See if she has suggestions -- if she's been doing it for any length of time she'll have seen all sorts of stalls and be able to make suggestions.  (or he, if your RD is male)
