I'm engaged, and my fiance and I live in our townhouse with our two energetic puppies. We drive his and hers Honda Civics, watch a lot of movies, and play video games (well, more accurately, HE plays video games, and I watch). I love music (alternative rock, classic rock, adult contemporary, emo, ska, pop-punk, golden oldies...), love to decorate, and love my family. I emjoy spoiling my two nieces and carrying on long phone conversations with my sister and parents, who all now live in Virginia Beach, where I hope to move to in a few years as well.
So, almost another whole month later... on November 10, 2008 1:20 pm
Alright. It's November. I've had my consultation with Dr. Schweitzer. Despite having been warned of his not-so-fabulous bedside manner, I actually really like him. I find him easy to talk to and very straightforward--I wasn't afraid to ask him anything at all.
The one minor speedbump I did hit is that he said there could possibly be an issue with scar tissue from a prior surgery (I had a left oopherectomy--left ovary removed due to large cyst (also removed)) that I had done open on my lower abdomen. He said if there is too much scar tissue, there is a possibility he may only be able to do the "sleeve" and not the "switch." After much freaking out, thinking, and talking it through with others, I've decided he just had to give me the worst case scenario to cover all his bases and that if others who have had prior WLS procedures, multiple C-sections, repaired hernias, etc., could get a full DS without incident, then so can I, dammit! And I will be sure to tell him so next time I see him 
I've also now had my psych eval. It was super easy, so I guess I'm officially not crazy. The psych was actually really enthusiastic about the DS, so that made it just that much easier. I highly recommend him to anyone in the NoVA area who neds the psych eval for insurance. It was Dr. Khot at Prince William Hospital in Manassas.
So, now I'm just waiting for the 18th when I have my final visit with my PCP to complete my 6 months of dieting for insurance. Once I complete that visit and get my letter of medical necessity from my doc, I should be all set to submit to insurance!
I'm impatient...time is ticking by slooooowly...

Neglecting to update for almost 2 months!
I'm still waiting, so not much to update, I suppose. My doctor and I looked back at some prior appointment records, and it looks like I'll be able to count my 6 months of dieting as having started in May instead of July since I saw her in May and June, and she documented my weight and that we discussed diet and exercise both times. That's another 2 months I don't have to wait!
I have my consultation with the surgeon on October 27 and will be going to the lab at Johns Hopkins directly afterwards to met a woman running a WLS study that I may participate in. My mother will go with me to both appointments (coming from 200 miles away on a weekday--yeah, she loves me!).
I set up an appointment for my psych evaluation on November 3 using a psych closer to me to save myself another trip to Baltimore. Another woman I know in my area is using the same surgeon for the same surgery and used this psych, so all should be fine (the psych's office even verified they were in-network for my insurance).
I have another gastric bypass support group meeting tonight. I've been in contact with other DSers in the area, and some of them will be coming to the group tonight, too. If we have a dedicated group of at least 4 DSers established, the nutritionist who runs the group said she would be willing to help us start up a DSers' only group--awesome!
So, I feel I'm well on my way. I think I may be looking at surgery now around March 2009, assuming all goes well with insurance submittal and approval. I occasionally have feelings of defeat, like I should have been able to become healthy without surgery, but I never doubt my decision to have the DS as my WLS procedure.

Well, I am 95% sure I know all my insurance requirements now (thanks, Sean!). I need the following:
- typical age and BMI requirements
- "Documentation of successful completion of at least 6 consecutive months of supervised conservative weight loss program...Successful completion means weight loss or at least maintenance of initial weight prior to participation of the program..."
- "Documentation of participation in an organized multidisciplinary surgical preparatory regimen in order to improve surgical outcomes, reduce the potential for surgical complications, and establish the member's ability to comply with post-operative medical care and dietary restrictions. The regimen should provide guidance on diet, physical activity, and behavioral and social support prior to and after surgery."
- "Documentation of pre-operative psychological evaluation..."
And so, I am now into my second month of my required 6 months of dieting with my physician. I have attended 2 information sessions (offered by 2 different practices). My inital colsultation with Dr. Schweitzer is scheduled for October 27. I have met with a group of fabulous DSers in MD and VA. I have joined a local WLS support group. That "organized multidisciplinary surgical preparatory regimen" has me a bit confused, but hopefully the surgeon's office will know what that's all about.
So, I'm on my way! Assuming I get approved the first time I submit for insurance approval, I am estimating a surgery date in May or June 2009 (possibly later if appeals come into play for any reason).

Yup. Waiting. Fun, right? I think my insurance requires 6 months of medically supervised diet, though I'm not 100% sure since I can't seem to get any answers from my insurance company. If so, I would like to set up my first appointment with either my doc or a nutritionist. But I won;'t do that until I am sure it is required. The more I read and ponder, the more excited I get. I'm very confident that this is the right decision for me. That says a lot since I can be very wishy-washy about a lot of things!

Well, I talked to my doctor, and she is completely supportive. I read about some surgeons, and I think Michael Schweitzer in Baltimore is going to be my first choice since he has a great reputation, low rate of complications, and performs DS laparoscopically, which I'm really hoping for. I checked with my insurance company, and he is considered an in-network surgeon, and Johns Hopkins is an in-network facility (which is where he practices).
I also found a form online from my insurnce company that outlines the requirements for qualifying to have the surgery covered. I called a doctor from a few years ago to get my medical records to show that I have been obese fr at least 5 years since that is one of the requirements. I will also be working with my doc to show 6 months of attempted weight loss on a specific plan (South Beach, which I know she knows I was doing since before I even started seeing her, so hopefully I can start the 6 months counting from my first appointment with her, which was about 2 months ago).
My sister's husband said I should do the band instead of a full surgery. I quickly spouted off MANY reasons why the DS is a better choice for me. I'd rather start with a surgery that is more likely to be successful than have to go back for revisions later on. I also want the surgery that offers the highest chance of curing my diabetes. Oh, AND the DS has the best chance of maintaining the lost weight long-term.
I registered for an information session with Johns Hopkins in early August. Jeff will go with me, and his cousin has expressed interest in going with me as well. Hurray for supportive family!

My Story
I was diagnosed with Type 2 Diabetes in May 2008 at age 25. I've been overweight my whole life, and I'd always said I didn't want to try WLS because I didn't have any weight-related health problems, so I would just keep trying to lose the weight on my own. Well, with the diagnosis of diabetes also came elevated triglycerides and high blood pressure. I was no longer fat-but-healthy. Something needed to be done.
