Am I overreacting ?

In waiting for my surgeon to submit my paperwork to my insurance company. I have Humana platinum PPO. I know I have the requirements completed but when I gave information to the girl who submits the  insurance claims she sort if looked like you only went to the dr. Twice for Weightloss but I couldn't get a receipt from my personal trainer or statement that said " she paid me $1500 to lose 7lbs". I got my insurance because you don't have all those requirements like other insurance companies.  I'm just venting and of course I'm scared and anxious.
