Hi Amy, I hope you get through this part of the journey. When my insurance needed a diet history. I thought I was doomed. I had NO medical supervised diets. I had no over the counter diet .Went to NO gyms, nothing. All my diets where what I started on my own. So what I did was make up one. I noticed on the diet sheet. They wanted list of Drs and dates, how much you lost. I was totally upset until I figured it out. THEY DIDN"T ASK FOR ADDRESSES OF THE DRS. So how was they going to verify what I say?I filled out the sheet looked up fad diets etc. There was no problem it was sent to my insurance and I was approved in a couple days. I know it's not good to lie. But for them to ask for such a thing is dumb to me. everyone don't have a medical supervised diet especially years of them. Hugs
