How long have all these approvals had the ins?

I am a business owner and am buying ins for my group. I am interested in the surgery but am wondering if they will deem pre-existing since the group plan is new. Does any one have a similar situation? Has anyone signed on for the UHC ins and gone for surgery within a short period?
