3 month aetna program, and how long before approval?

i am 2 weeks shy of finishing my 3 month nutritionist progrma for aetna. the office of the surgeon i am going to has said that aetna should approve it with no problem. i wanted to know if anyone has been approved for the 3 month program, and also how long it took for the insurance company to approve or deny? ALSO how long does an appeal take? THANK YOU!
