Blue Advantage in Iowa

Anyone been approved thru Blue Advantage in Iowa? The insurance company said that if the surgery was medically necessary they would approve it. I go to my first appt. with my primary physician this Friday to see about referring me to a surgeon. I have a BMI of 42. 5'8 280 pounds. I have stress incontinence, not taking medication but I have a blood pressure of 140/90, heartburn, family history of diabetes, strokes and congestive heart failure. Would I be approved?
