5 yr history of MO-- any 5 yrs?

My insurance states "at least a 5 yr history supported by documentation." It doesn't say the "LAST" 5 years, and it doesn't say consequetive 5 years. I lost weight after a spinal cord injury and surgery in 2002. I wasn't dieting-- it was a side effect of my condition. I've gradually put the weight back on in the meantime--trying lots of diets trying to stall the regaining. I guess it helped a little, but here I am--back again. I have my med records for 1996-2001 showing MO. I have even more years, but was in graduate school and the student health center shreds them after 10 yrs. So when I requested them a few months ago (2007), my last time with them was 1996, so I just missed it. They're gone. Does insurance (mine is NJ BC/BS) require it to be the most recent 5 yrs or just any? Can it be cummulative? I may only have 1 weight recorded for one of those 5 yrs (2001) and they may not accept that. However, I've been MO for the last 6 months (despite my efforts to diet). Counld they add all my revent weigh ins with the 4+ years from before? Thanks, Kahlua
