anyone have medical metual (supermed)

Has anyone recently applied for medical Mutual (supermed) coverage and if so what was the outcome. What did you need to provide to them, how long did it take to hear back, and if you were denied what was the reason. Also, if you have any advice for me I would appreciate it. I am in the process of gathering all my medical records. Thanks to all Maris - Ohio
