Has anyone been approved through Bright Medical with a BMI of 40 and no other

Anyone with Bright medical/Cigna HMO, BMI of 40 and no other co-morbidities been approved? I have been doing everything they ask for 7 months and now they are giving me a tough time because I don't have diabetes, sleep apnea, high blood pressure, etc. Please help me with the approval process.
