What should a dietician put in her letter to the insurance company?

Hi Everyone My dietician whom I've been working with has finally agreed to write me a letter to my insurance carrier supporting my decision for RNY. We're wondering if there is anything specific she should put in it. Anything that may or may not push the insurance companies decision in my favor? Any and all thoughts or ideas would be greatly appreciated. Thank you.
