I know this may sound stupid but I need to know what to say when I call???

I am wanting to know just what exactly what to say when I call my ins. commissioner. My policy w/ my ins. is medical neccessity and I was denied. The surgeon's office said they felt that they proven it was medical neccessity. Please if anyone has contacted your ins. commissioner let me know what you told them? This will be a great help to me! I live in N.C.
