Does anyone have a list of questions I should have to ask my Dr at first visit?

My Dr. Does not offer any information. But he is real good about answering any questions asked. I would like a list of things to ask him so I don't get home and think of somehting I wish I had asked. I would very much appreciate any ideas you would have for me in obtaining a list of questions to ask him. Thanks e-mail [email protected]
