How strict are his guidelines for WLS candidicy?

I have read the UC Davis web site concerning bariatric surgery guidelines, but I was wondering if those who have had surgery by Dr. Wolfe have had to actually have documented medical supervision (doctor supervised medication, nutritional studies, etc...) before he classified you as a candidate. After fighting this weight problem alone for so long, will I have to go through months or years of the same thing with the doctor watching me?
