What will I need for Indiana Medicaid for precertification?

I am seeing my surgeon for the first time on April 5th and I want to have everything that medicaid will need ready for him to send. I already have had PCP, sleep center, GI doc, psych doc, nutrionist, back doc and chiropractor send records/letters to surgeon. Just don't know what medicaid is going to want. Any help is greatly appreciated.
