Does anyone have any information about the appoval process?

I've noticed that Kaiser Permanente has different criteria and requirements depending on where they are. Northern Cal has a set of rules, So. Cal- a different set, and other states, a whole different set of requirements. I am trying to have the surgery performed w/ Kaiser (my carrier), but would like to know what I am up against....please email me or post here with any information....no matter how small. Thanks
