Exclustion says will pay for endogenous obesity and not exogenous obesity?

Can someone please help me understand. I know endogenous means from within and exogenous is from outside. My insurance company denies me with a letter stating "Based on the medical documentation submitted, Misty's condition does not meet the definition of endogenous obesity. Therefore, we are denying benefits for the surgery based on the exclusion." So what is the definition and criteria for endogenous obesity. I have already sent them a letter asking that question and am now awaiting there response which has already been a week. I would like some input from anyone please... God Bless
