can someone who appealed with bsbs Fl ppo please tell me?

where do you send the appeal to?I sent out my appeal yesterday I sent it to the address it has on back of explaination of benefits it has an address on there that says ppo appeal dept. so that is where I sent it my surgeons office told me I have to send the appeal on my own I just hope I sent it to the right place and if anyone knows is it Ok. to send the appeal myself eventhough my surgeon sent the first request for approval I am just alittle nervouse thank you for any help...
