I HATE BCBS of Kansas!! Need help with letter to them.

I called my insurance company BCBS of Kansas and they have told me that I have an exclusion in my policy and that they do not pay for weight loss surgery. I have nothing to lose so I would like to send a letter to them to try to get coverage. I have no idea where to start. If anyone has any ideas, please let me know. Thanks.
