Where can I find ICD codes?

I am basically trying to fight my own battle with insurance. (I have state insurance and my surgeon's insurance people doubt they'll cover it so they haven't been very supportive) I received yet another suspended letter from my insurance today and they've requested ICD codes for all of my co-morbidities. I have seen them posted somewhere before, but can't find them (I know I know I should've marked it when I seen it)
